The patient is insured with Just (CZ label) and has identified ADHDcentraal and Forta Bosman GGZ / Dokter Bosman as providers with evidence of a contracted route. Please consider the shortest clinically appropriate location/age pathway and include the suspected diagnosis, relevant history, medication and risk assessment in the referral.

Before sending a referral: Contract status can vary by the exact legal entity, location, care type and budget cap.
